ECM/CS

Heritage Clinic is all an Enhanced Care Management (ECM) and Community Supports provider. ECM and CS are foundational parts of the Cal AIM transformation of Medi-Cal focused on:

• Breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities;
• Introducing a better way to coordinate care; and
• Providing high-need members with in-person care management where they live (DHCS website)

Heritage Clinic is contracted for ECM and CS service with Anthem, Health Net, LA Care, and Molina. Persons with those managed care plans needing case management assistance, housing deposits, housing sustaining, or housing tenancy services may be eligible.
