Outpatient Care Services Program

Heritage Clinic provides mental health services for adults 55+ as well as some younger adults with physical challenges. In partnership with the Los Angeles County Department of Mental Health, these services are delivered through a Recovery Resilience Reintegration Program for Older Adults Program. The Clinic extends its field-based mental health services to seniors in a partnership between the older adult and clinical team, to support older adults with mental health difficulties. These services include psychotherapy, counseling, case management, advocacy and psychiatric medications. These services are employed to help older adults who may be experiencing one of the following:

• Hospitalization or risk of acute psychiatric hospitalization
• Risk of premature placement at a Skilled Nursing Facility / nursing homePresence of co-occurring substance abuse disorder
• Risk of abuse or self-neglect
• Risk of harm to others
• Risk of suicide

Services Include:

• Outreach in the community
• Engaging those who might be reluctant to receive services
• Assessment
• Case management
• Medication evaluation and follow-up
• Psychotherapy and Counseling
• Crisis intervention and management
• Peer advocacy and support
• Client and caregiver support groups
• Neuropsychological evaluation

Payment for services:
Services are available for people who meet the eligibility criteria, as determined by the Department of Mental Health. Individuals with Medi-Cal, Medicare/Medi-Cal, (this includes all the Cal Medi-Connect Health Plans) or who are low income with no insurance, are eligible.
