Full Service Partnership Program

Heritage Clinic provides intensive mental health services for adults 55+ as well as some younger adults with physical challenges. In partnership with the Los Angeles County Department of Mental Health, these services are delivered through a Full Service Partnership for Older Adults Program. The Clinic extends its field-based mental health services to seniors by providing “whatever it takes,” as decided in partnership between the older adult and clinical team, to support older adults with the most serious mental health difficulties. These services include psychotherapy, counseling, case management, advocacy and psychiatric medications. These specialized services are employed to help older adults whose independence has been jeopardized or is at risk of being jeopardized by at least one of the following:

• Homelessness or risk of homelessness
• Hospitalization or risk of acute psychiatric hospitalization
• Incarceration or risk of going to jail
• Risk of premature placement at a Skilled Nursing Facility / nursing home
• Presence of co-occurring substance abuse disorder
• Risk of abuse or self-neglect
• Risk of harm to others
• Risk of suicide

Services Include:

• Outreach in the community
• Engaging those who might be reluctant to receive services
• Assessment
• Case management
• Medication evaluation and follow-up
• Psychotherapy and Counseling
• Crisis intervention and management
• Peer advocacy and support
• Client and caregiver support groups
• Neuropsychological evaluation

Payment for services:
Services are available for people who meet the eligibility criteria, as determined by the Department of Mental Health. Individuals with Medi-Cal, Medicare/Medi-Cal, (this includes all the Cal Medi-Connect Health Plans) or who are low income with no insurance, are eligible.
